While most issues between client and clinician can be addressed directly, some situations may require escalation. In compliance with the State of Texas, we provide information below to assist your pursuit of a report, if you should find yourself in such a situation.
Alternatively, some may have had wonderful experiences with their clinician and like to show support for their practice. Google reviews may be left for here.

Business Violation Concerns
If you believe you have been a victim fraudulent business practices in the State of Texas please contact the Attorney General's Office to file a consumer complaint.

Records Request
If needed your client records can be requested via the portal or by emailing our office info@compassioncounseling.us
To help prevent delays, please include the following information with your request:
• Client full legal name and date of birth
• Current phone number and or email address
• Description of records requested including specific dates if applicable
• Preferred method of delivery such as secure electronic delivery, mail, or in person pickup
• Client signature and date
If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.
Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.
